This form is provided by Bristol Home Education so that parents know exactly what is involved when dealing with the local LEA - this form is in no way a legal requirement and parents do not have to fill it in. If you are in any doubt please contact info@bristolhomeeducation.org.uk

	The Local Education Authority has a legal duty to satisfy itself that children being educated at home receive ‘efficient, full-time education suitable to their age, ability and aptitude’. To help us in this, please fill in this form to give us some details about your child and what you are looking to achieve in educating him or her at home. Please complete a form for each child concerned.
